If you are unable to report to work due to sickness, you or someone else on your behalf must inform the manager on or before your first day of sickness absence, preferably by telephone before 9 am. The reason for and likely duration of your absence must be given.

Periods of sickness up to and including 5 working days are to be evidenced by the completion of a self-certificate, which must be received upon or before your return to duty.

Periods of sickness exceeding 5 working days are to be evidenced by a certificate from your doctor or hospital covering the period of incapacity until declared fit to return to work. You must keep your manager regularly informed of your absence and likely duration of your absence. Further medical certificates are required for each further working week of sickness absence. Failure to notify the Employer of your absence may render you subject to disciplinary action and may also bar you from receiving statutory sick pay.

The Employer does not operate a sick pay scheme. Your entitlement is to statutory sick pay only. The Employer will carry out its obligations to pay Statutory Sick Pay (SSP) subject to current legislation and subject to you following the above procedures.

You are required to disclose to the Employer details of any disability or medical condition known to you at the commencement of your employment and of any subsequent change(s) in your health which is/are likely to affect the performance of your obligations under this contract.

On return to work after a period of illness a meeting must be held between you and your manager to assess your fitness to work, any staged return requirements or any change in working processes required to ensure that equality and diversity requirements are satisfied. If required a specific risk assessment shall be undertaken to ensure that appropriate control measures are in place. Risk assessments shall be revisited as your condition improves/deteriorates and specialist advice from your doctor or occupational health to meet your needs.
